There will be two categories of additional content added here.
Firstly, the parts indicated as cut out of the final Chapter 16 (due to the book becoming a bit too fat and that content being either duplicative of Chapter 3 or probably of less interest to most readers).
Secondly, updates and further thoughts as I find time to add them in. (I already have plenty of further thoughts but deciding how to present them has to be done before putting them out here.)
